Request coupons now for the upcoming Ringling Brothers and Barnum and Bailey Circus. From 11/06 to 11/16 at the Allstate Arena and from 11/18 to 11/30 at the United Center. All tickets are $9 with a coupon. Limited to two tickets per coupon. Not valid for VIP and Circus Celebrity Seats on the 9th and 23rd of November or 5pm shows. Clarification of IDHS Child Care Collaboration Program Issues

2008-09 Medical, Mental Health, Dental and Nutrition Cooperative Agreements with supporting provider credentials are due into CYS no later than Monday, November 17, 2008.

A CYS review signature will not be required prior to contracting with your provider. It is expected that delegate agencies will contract with professionals holding current license/certification in their respective discipline and have experience in working with the unique needs of the 0-5 population.

If you need further guidance on provider qualifications, feel free to contact your assigned CYS Health Support Services Coordinator (medical and mental health) or CYS Public Health Nutritionist III (dental, nutrition).
